This program is designed to assist nursing home residents who want to return to community living. Our Transition Navigator will work with you and your informal support system to help you develop plans for housing, services and support. You must be currently in a nursing home and have Medicaid in order to qualify for this program.

Services include:

  • Help with reviewing your options for care in the community
  • Housing search and assistance
  • Coordination of goods and services for community living
  • Connection to community services that best fit your needs

    Refer Someone

    Referring someone to the Area Agency on Aging is easy and can make a real difference in the lives of older adults and people with disabilities in your community. If you know someone who could benefit from their assistance, simply provide us with their contact information and we'll take it from there.
