The WellWise Services Area Agency on Aging, established as a private non-profit in 1974, actively administers the MI Choice Medicaid Waiver program. WellWise Services extends its services to Hillsdale, Jackson, and Lenawee counties. Eligible adults who meet Nursing Facility Level of Care (NFLOC), income, and asset criteria can actively access Medicaid-covered services akin to those provided by nursing homes. This choice empowers them to remain in the comfort of their own home or select an alternative residential setting. In this program, each participant actively receives the fundamental services covered by Michigan Medicaid, including supports coordination and one or more distinct services tailored to their specific needs and preferences.

Interested in Enrolling in the WellWise Services Area Agency on Aging Network? You Will Need To:

  • Meet the MI Choice Waiver program requirements.
  • Complete the Provider Enrollment Application.
  • Attend semiannual provider training meetings.

This contact will facilitate discussions about the MI Choice Waiver Provider Enrollment requirements and the process for requesting the Provider Enrollment Application.

It’s worth noting that we closely monitor compliance with the Conditions of Participation and Service Standards through onsite reviews of our providers. In cases where providers demonstrate areas of non-compliance with these requirements, they may be placed on probationary status or, in more severe cases, have their Agreement with WellWise Services Area Agency on Aging terminated.

To maintain the highest quality of care for our program participants, we maintain a dedicated pool of quality providers. This ensures that we have the necessary service capacity to meet the needs of those seeking waiver services.
