Doing Business With Us
The WellWise Services Area Agency on Aging, established as a private non-profit in 1974, actively administers the MI Choice Waiver program in Hillsdale, Jackson and Lenawee Counties. Eligible adults who meet Nursing Facility Level of Care (NFLOC), income, and asset criteria can access Medicaid-covered services. The MI Choice Waiver Program empowers them to remain in the comfort of their own home or select an alternative residential setting. In this program, each participant actively receives the fundamental services covered by Michigan Medicaid. This includes support coordination and one or more distinct services tailored to their specific needs and preferences.
Interested in becoming a Provider with WellWise Services Area Agency on Aging? You Will Need To:
- Meet the MI Choice Waiver program requirements.
- Complete the Provider Enrollment Application.
- Attend semiannual provider training meetings.
