Services that help you stay safe and independent at home.

Home and Community-Based Services include MI Choice Waiver and Tiered Care programs. These programs offer a realistic choice for receiving care in your home and community rather than in a nursing home. Contact us to see if you qualify.

MI Choice Waiver

MI Choice Waiver is a community based, in-home services program offering you the choice to remain independent and safe in your home and community. Additionally, this program is designed for adults 65 and older and adults 18 and older with a disability who are medically and financially eligible.
