Helping Residents in Nursing Facilities Return to Community Living

Eligibility Requirements:

Must be in a skilled nursing facility, receiving rehabilitation or long-term care.

Must have active Medicaid or be eligible for Medicaid, or have all of the following:

  • A completed Michigan Medicaid application with all necessary verifications submitted to the local MDHHS office awaiting review;
  • Reasonable assurance that the local MDHHS office will likely approve the submitted application; and
  • Application registration on Bridges as verified by MDHHS

Services include:

  • Options counseling and navigation
  • Housing search and assistance
  • Coordination of goods and services for community living
  • Connection to community services that best fit your needs
