MI Choice Waiver
MI Choice Waiver is a community based, in-home services program offering you the choice to remain independent and safe in your home and community. Additionally, this program is designed for adults 65 and older and adults 18 and older with a disability who are medically and financially eligible.
Overall, this program is a supplement to family, friends, neighbors and other caregivers that make up the informal support system. In summary, speak to one of our Home and Community-Based Services experts to find out if someone is eligible.
MI Choice Waiver Services
Eligibility Requirements:
- 65 and older, or 18 and older with disability
- Need nursing home level of care but living in a community setting such as a house, apartment, or assisted living
- Meet MI Choice program and Medicaid eligibility requirements
- In need of services and supports to help you at home and in the community

Nurses and Social Workers will:

Home and Community- Based Services: Tiered Care Program
The Home and Community-Based Services offer a Tiered Care Program. In other words, this is a comprehensive, community based, in home program that allows you to remain independent and safe in your home and community. Above all, Tiered Care is designed to help those who are 60 years or older, medically complex or in need of supportive services. This program was created to help those remain living in the community. There are not any income eligibility requirements for most Tiered Care services.
